Failure Analysis and Prevention

Understanding how and why a piece of equipment or component fails requires a combination of experience, skill and knowledge.  Determination of the Root Cause of a failure includes combinations of Laboratory Testing (Metallographic Analysis, Chemical Analysis, Physical Property Testing), Fractography, Scanning Electron Microscopy, On Site Investigation, Design Review, Equipment History Review and Engineering Calculations.   Once the Root Cause has been determined the appropriate repair can be implemented.
